Demographics
Patient name, address, phone number etc., for identification and matching.
Diagnosis/Complaint
The reason the patient initially sought care and/or the diagnosis made by the clinician treating the patient.
Allergies
A list of medications or other items to which the patient is believed to be allergic.
Inpatient Medications
Medications given to the patient while the patient was admitted to the hospital.
Outpatient Medications
Medications that are prescribed by a clinician for the patient to take outside the hospital.
Patient Consent
The status of a patient’s decision to grant or deny an institution access to their health information available via HIXNY’s Health Information Exchange. The types of consent status:
  1. Granted: Allows clinicians for the designated institution to view a patient’s health information available through the Health Information Exchange.
  2. Denied: Prevents clinicians for the designated institution from viewing a patient’s health information, even in the event of an emergency.
  3. Null (Neither Granted nor Denied): Allows clinicians to access a patient’s health information ONLY in the case of an emergency.
Clinical Encounters
A list of the clinicians currently and recently providing care to the patient, including the dates of patient visits to these clinicians.
Lab Results
Results of laboratory tests ordered by a patient’s physician.
Radiology Reports
Results of radiology tests ordered by a patient’s physician.
Physician Notes
Notes taken by the patient’s clinician during recent visits.
Discharge Summaries
Summary of follow up care recommended by a hospital after a patient is discharged.
