– Both organizations help underserved populations in the North Country. –

ALBANY, N.Y. – Hixny connected two organizations to the community-wide health information exchange (HIE) in November, further expanding the availability of patient records to the statewide healthcare community and to individual healthcare consumers.

The new connections are:

  • Mountain Lake Services, a human services organization serving individuals with intellectual and developmental disabilities in Essex County as part of The Arc New York.
  • Fort Hudson Certified Home Health Agency, serving seniors in Warren, Hamilton, Washington, Saratoga and Essex counties.

Both Mountain Lake Services and Fort Hudson Certified Home Health Agency are contributing data using Consolidated-Clinical Document Architecture (C-CDA), the industry standard format for exchanging clinical information.
